Integrated Models of Care: Australian Examples

The prevalence of malnutrition and its negative consequences in patients with cancer is well documented. Implementing an oncology outpatient model has been shown as an effective strategy to reduce malnutrition prevalence, improve clinical outcomes and support the medical system. However, the adoption of integrated models of care in outpatient settings has been limited and requires additional implementation and evaluation. Several innovative models of care utilizing medical nutrition therapy have been implemented and evaluated in Australian’s cancer centres to provide rehabilitation and survivorship care in the community. Positive outcomes have been observed including lower risks of malnutrition, improved adherence to evidence-based nutrition guidelines, reduced post-operative length of hospital stay, reduced health services expenses, and increased patients’ and clinicians’ satisfaction. This presentation will introduce the key components of these models of care in outpatient settings and suggest future directions for implementing an effective model of care for cancer patients in the community.

Dr. Liz Isenring is passionate about improving lives through evidence-based nutrition. Dr. Isenring is the Director of LINC Nutrition, Adjunct Honorary Professor at Bond University, an international speaker, author of over 160 peer-reviewed scientific papers, and has received over 4 million dollars in research support. She has held professor, head of program, associate dean of research, editorial, and professional leadership roles.
